The Child and Family Guidance Center in Norwalk, CT is seeking an Overnight Case Manager to work overnight 9 PM–6 AM, Monday through Friday. The role focuses on coordinating services for children ages zero to 18 in crisis and mitigating the need for hospital visits by arranging appropriate supports.

The successful candidate will have a Bachelor’s in Social Work and relevant experience, with bilingual Spanish preferred and a valid driver’s license for local travel within Bridgeport/Norwalk.

Qualifikationen

• Bachelor's degree in Social Work required or similar.
• 2+ years’ experience with children and families with emotional/behavioral difficulties preferred.
• Bi-Lingual Spanish preferred.
• Must have driver’s license and access to a car; able to travel within Bridgeport/Norwalk area.

Aufgaben

• Assist families in obtaining and securing necessary services (transportation, translation, advocacy, educational and social supports).
• Collaborate with MCIS clinicians to link families to follow-up services.
• Provide mobile crisis services with MCIS clinician.
• Outreach to community agencies, schools, courts as needed.
• Participate in weekly supervision and team meetings; maintain records and data.
