Job summary

Community Human Services (Bridge Housing) seeks a part-time Community Support Specialist to provide case management and supportive services in a 24/7 residential setting. You will work overnight, maintain documentation, build therapeutic relationships, and coordinate with the team on individualized housing and life-skills goals.

Responsibilities include crisis management, safety checks, and community resource navigation, with an emphasis on Harm Reduction and Housing First models.

Qualifications

• High school diploma required.
• Two years’ work experience in case management or equivalent, per agency/funder.
• Act 34/Act 33/FBI clearances required on hire.
• Comprehensive Crisis Management (CCM) and First Aid/CPR/AED certifications or willingness to obtain.

Responsibilities

• Develop professional therapeutic relationships with residents.
• Provide direct case management and assist with benefits and resources.
• Conduct safety checks inside and outside facility as directed.
• Distribute meals and maintain safety and hygiene.
• Document daily progress notes and update case records.
• Coordinate with the team to set housing and employment goals.
