Job summary

Sunrise Senior Living is seeking a Wellness Nurse LPN to monitor resident health, support medication oversight, and uphold clinical quality across the community. The role combines proactive assessment with strong clinical oversight to prevent decline and ensure resident wellbeing.

You will conduct regular health assessments, transcribe orders accurately, and collaborate with families and physicians to deliver compassionate care in a supportive environment. Competitive benefits accompany base pay.

Qualifications

• Active LPN license in good standing.
• Graduate of approved nursing program.
• 1+ year experience in senior care or clinical setting.
• Strong assessment, documentation, and communication skills.

Responsibilities

• Conduct move-in, 3-day, semi-annual, and change-of-condition assessments.
• Perform monthly wellness visits and monitor health trends.
• Follow up on falls, incidents, and hospital returns.
• Transcribe physician orders and ensure accurate MAR/TAR documentation.
• Complete medication audits and oversee medication compliance.
• Partner with families, physicians, and the Resident Care Director.
• Support infection control and regulatory standards.
• Educate and guide care staff on documentation and medication practices.

Pay Range:

35.10 - 46.85
