Job summary

Seven Hills Foundation is seeking a Direct Support Professional II for asleep overnight shifts. You’ll empower individuals to live meaningful, independent lives in their community while providing daily care in a residential setting.

You will follow ISP and Behavior Support Plans, assist with personal care and medications, and support clients in community activities. Leadership, teamwork, and strong communication are essential to success.

Qualifications

• Experience delivering direct support in residential settings or community-based programs.
• Ability to follow ISP/Behavior Support Plans and document progress.
• Strong communication, teamwork, and problem-solving skills.
• Commitment to dignity, respect, and empowerment of individuals.

Responsibilities

• Provide daily support and care to individuals in a residential setting.
• Help individuals build independence by teaching life skills at home and in the community.
• Follow and implement Individual Support Plans (ISP) and Behavior Support Plans.
• Assist with personal care, medication administration, and health-related needs.
• Support individuals in community activities, relationships, and social connections.
• Lead and mentor other staff when needed.
