Columbus House, Inc. is seeking a PATH Outreach & Engagement Case Manager in Middletown Township, PA.

This full-time, non-exempt role (32 hours/week) focuses on linking consumers to housing, education, employment, and health resources, with an emphasis on trauma-informed and gender-specific care. Responsibilities include street outreach, providing comprehensive case management in community settings, and collaboration with other outreach and shelter teams.

Responsabilidades

• Engage in street outreach to consumers who have histories of homelessness, mental illness, substance abuse, unemployment, incomplete educations, lack of employment skills and are disengaged from services.
• Provide a full range of case management services to individuals in the community, e.g. in soup kitchens, shelters and on the streets.
• Team with and support all other outreach case managers and shelter case managers as appropriate.
• Attend all required network and staff meetings; meet with Middlesex and Transitional Programs Manager for regular supervision; review caseload weekly; complete required monthly, weekly and daily charting and movement reports.
• Collaborate with the CAN Entry and Exit process in Middlesex region.
• Work one evening a week at the local Warming Center during winter season after 9PM.
• Complete all required documentation accurately and timely; enter all client data into Case Worthy software.
