Community Care Physicians is seeking an experienced Nurse Practitioner or Physician Assistant to join Vascular Health Partners in the Capital Region of New York. You’ll work with four board-certified vascular surgeons in an outpatient-focused practice dedicated to arterial and venous disease.

The role includes travel to satellite offices in Saratoga Springs, Queensbury, and Amsterdam, with a 4‑10 schedule and no overnight rounds.

Qualifications

• Current New York State licensure as a Nurse Practitioner or Physician Assistant.
• Graduate of an accredited NP or PA program.
• DEA registration (or ability to obtain).
• Strong clinical assessment and communication skills.
• Ability to work independently while collaborating closely with physicians.

Responsabilités

• Evaluate, diagnose, and manage arterial and venous disease in outpatient settings.
• Perform comprehensive vascular assessments and develop treatment plans with vascular surgeons.
• Provide pre- and post-operative evaluations and follow-up care.
• Review vascular ultrasound and diagnostic test results.
• Coordinate care with referring physicians and specialists.
• Educate patients on vascular disease prevention and treatment options.
