Job summary

Community Care Physicians is seeking an experienced Nurse Practitioner or Physician Assistant to join Vascular Health Partners, a premier vascular surgery practice in New York’s Capital Region. You’ll work with four vascular surgeons and an experienced APP team in a state-of-the-art outpatient suite with travel to satellite offices.

The role emphasizes evaluation, diagnosis, and management of arterial and venous diseases, with collaborative care and patient education.

Qualifications

• Current New York State licensure as a Nurse Practitioner or Physician Assistant.
• Graduate of an accredited NP or PA program.
• DEA registration (or ability to obtain).

Responsibilities

• Evaluate, diagnose, and manage patients with arterial and venous disease in the outpatient setting.
• Perform comprehensive vascular assessments and develop treatment plans with vascular surgeons.
• Coordinate care with referring physicians and other specialists.
• Educate patients regarding vascular disease prevention and treatment options.
