Job summary

Community Care Physicians is seeking an experienced Nurse Practitioner or Physician Assistant to join Vascular Health Partners in the Capital Region. You will work with four board-certified vascular surgeons and an experienced APP team, mainly from the Latham office and interventional suite, with travel to satellite offices in Saratoga Springs, Queensbury, and Amsterdam.

Qualifications

• Current New York State licensure as a Nurse Practitioner or Physician Assistant.
• Graduate of an accredited NP or PA program.
• DEA registration (or ability to obtain).
• Strong clinical assessment and communication skills.
• Ability to work independently while collaborating closely with physicians.

Responsibilities

• Evaluate, diagnose, and manage patients with arterial and venous disease in outpatient setting.
• Perform comprehensive vascular assessments and develop treatment plans in collaboration with vascular surgeons.
• Review vascular ultrasound and diagnostic testing results.
• Coordinate care with referring primary care physicians and specialists.
• Educate patients regarding vascular disease prevention, treatment options, and lifestyle modifications.
