Job summary

Community Care Physicians seeks an experienced Nurse Practitioner or Physician Assistant to join Vascular Health Partners in the Capital Region of New York. You will work with four board-certified vascular surgeons and a team of APPs dedicated to arterial and venous disease, performing outpatient assessments and endovascular care without overnight hospital rounds.

Qualifications

• Current New York State licensure as a Nurse Practitioner or Physician Assistant.
• Graduate of an accredited NP or PA program.
• DEA registration (or ability to obtain).

Responsibilities

• Evaluate, diagnose, and manage outpatient arterial and venous disease.
• Perform vascular assessments and develop treatment plans with vascular surgeons.
• Coordinate care with referring primary care physicians and specialists.
• Educate patients regarding vascular disease prevention, treatment options, and lifestyle changes.
• Review vascular ultrasound and diagnostic testing results.
