Job summary

Community Care Physicians is seeking an experienced NP or PA to join Vascular Health Partners, a premier vascular surgery practice in the Capital Region of New York.

You will work with four board-certified vascular surgeons and a team of APPs in a state-of-the-art outpatient setting, with travel to satellite offices. The practice emphasizes minimally invasive vascular care, education, and patient-centered outcomes.

Qualifications

• Current New York State licensure as a Nurse Practitioner or Physician Assistant.
• Graduate of an accredited NP or PA program.
• DEA registration (or ability to obtain).
• Strong clinical assessment and communication skills.
• Ability to work independently while collaborating with physicians.

Responsibilities

• Evaluate, diagnose, and manage outpatient vascular patients.
• Perform comprehensive vascular assessments and plan with surgeons.
• Manage PAD, carotid disease, aneurysms, DVT, and venous disease.
• Provide pre- and post-operative evaluations and follow-up care.
• Review vascular ultrasound and diagnostic testing results.
• Coordinate care with referring physicians and specialists.
• Educate patients on vascular disease prevention and lifestyle options.
