Job summary

Community Care Physicians is seeking an experienced NP or PA to join Vascular Health Partners in the Capital Region of New York. Work with four board-certified vascular surgeons and a team of APPs in a growing outpatient vascular program.

The role focuses on evaluation, diagnosis, and management of arterial/venous disease with travel to satellite offices for outpatient care, no overnight hospital rounds, and a strong emphasis on minimally invasive vascular procedures.

Qualifications

• Current New York State licensure as a Nurse Practitioner or Physician Assistant.
• Graduate of an accredited NP or PA program.
• DEA registration (or ability to obtain).
• Strong clinical assessment and communication skills.
• Ability to work independently while collaborating closely with physicians.

Responsibilities

• Evaluate, diagnose, and manage patients with arterial and venous disease in the outpatient setting.
• Perform comprehensive vascular assessments and develop treatment plans with vascular surgeons.
• Manage conditions including PAD, carotid disease, aneurysms, venous disease, DVT, and PVD.
• Provide pre-operative and post-operative evaluations and follow-up care.
• Review vascular ultrasound and diagnostic testing results and coordinate care with referring physicians.
• Educate patients on vascular disease prevention, treatment options, and lifestyle changes.
