Job summary

Community Care Physicians is seeking an experienced or highly motivated Nurse Practitioner or Physician Assistant to join Vascular Health Partners, one of the Capital Region’s premier vascular surgery practices.

You will work alongside four board-certified vascular surgeons and an experienced team, with an emphasis on minimally invasive endovascular procedures and comprehensive vascular care in NY's Capital Region.

Qualifications

• Current New York State licensure as a Nurse Practitioner or Physician Assistant.
• Graduate of an accredited NP or PA program.
• DEA registration (or ability to obtain).
• Strong clinical assessment and communication skills.
• Ability to work independently while collaborating closely with physicians.

Responsibilities

• Evaluate, diagnose, and manage patients with arterial and venous disease in the outpatient setting.
• Perform comprehensive vascular assessments and develop treatment plans in collaboration with vascular surgeons.
• Review vascular ultrasound and diagnostic testing results.
• Coordinate care with referring primary care physicians and specialists.
• Educate patients regarding vascular disease prevention, treatment options, and lifestyle modifications.
• Provide pre-operative and post-operative evaluations and follow-up care.
