Lancaster Behavioral Health Hospital (LBHH) is hiring a full-time Outpatient Therapist to join our inpatient and outpatient care team in Lancaster, PA. The role involves psychosocial assessments, treatment planning, and delivering individual, group, and family therapies to adults and adolescents with co-occurring disorders.

Regular team meetings and coordinated aftercare planning are essential components of the position.

Qualifications

• Licensure required: LPC, LSW or LCSW.
• MSW or master’s degree in a recognized mental health field.
• Two years of partial hospital experience.

Responsibilities

• Conduct psychosocial assessments to document patient history and identify treatment focus.
• Develop and implement treatment plans with concise, realistic care goals.
• Provide individual, group and family therapies using multiple modalities.
• Document treatment in the medical record in a timely, legible manner.
• Attend regular treatment team meetings to coordinate case management.
• Coordinate individualized aftercare plans leveraging family and community resources.
