Cedar Hills Hospital is seeking an Outpatient Program Therapist to coordinate discharge planning and develop post-hospitalization care plans. You will manage a 20-patient caseload, lead group therapies for 20 hours weekly, and conduct individual sessions for treatment planning, aftercare, and crisis intervention.

Required: Master’s degree in a related field, 1+ year behavioral health experience preferred, and licensure (LPC/LCSW/LMFT) preferred.

Competenze

• Master’s Degree from an accredited college or university in Social Work, Counseling, Psychology, Marriage and Family Therapy, or equivalent is required.
• Preferred: 1+ years in behavioral health.
• Clinical licensure (i.e. LPC, LCSW, LMFT) is preferred.
• CPR certification, Handle With Care and Verbal De-Escalation training within 30 days of employment; competency exam on hire.

Mansioni

• Plan and coordinate discharge planning activities.
• Develop and implement a plan of care for post-hospitalization needs to ensure continuity of care.
• Maintain a caseload of 20 PHP/IOP patients; conduct group therapy for 20 hours per week.
• Meet individually with clients for treatment planning, aftercare planning, and crisis intervention.
