Job summary

• Documents patient care provided.
• Monitors and communicates patient condition to care team, physician, patient, and family.
• Assists patients with activities of daily living (hygiene, elimination, nutrition, ambulation).
• Coordinates patient care with transfers, discharges, referrals, and psychosocial needs.
