Job summary

Qualifications

• LMSW or higher licensure required; LCSW preferred.
• Three or more years hospital social work or protective services experience preferred.
• Experience in psychosocial and therapeutic counseling preferred.
• Working knowledge of payors and reimbursement.
• Strong written and verbal communication; professional maturity.
• Basic understanding of Medicare, Medicaid, and Managed Care regulations.
• Organized and efficient in establishing priorities under timeframes.
• Self-motivated with minimal supervision.
• Confidential handling of verbal and written information; familiarity with community resources.

Responsibilities

• Intervenes with patients who have complex psychosocial needs and assists with eligibility for social programs.
• Offers crisis intervention to patients and families with psychosocial needs.
• Helps coordinate a plan of care for high‑risk patient populations.
• Receives referrals from at‑risk populations from interdisciplinary team members.
