Job summary

Qualifications

• LMSW or higher licensure is required; LCSW preferred.
• 3+ years hospital social work or protective services experience preferred.
• Experience in psychosocial and therapeutic counseling.
• Working knowledge of Medicare, Medicaid, and Managed Care regulations

Responsibilities

• Intervene with patients who have complex psychosocial needs.
• Assist with eligibility determinations for social programs and funding sources.
• Provide crisis intervention to patients and families and coordinate a plan of care for high-risk populations.
• Receive referrals for individuals from at-risk populations from interdisciplinary team members
