Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. The RN III will lead with advanced care coordination, patient education, and transitions of care while supporting a collaborative network of providers and care managers.

Ideal candidates have a BSN or accredited RN program graduate, Michigan RN license, CPR certification, and at least 3 years of ambulatory care experience.

Qualifikationen

• BSN strongly preferred or graduate of an accredited RN program; advanced certification/training preferred.
• Valid, unrestricted RN license in Michigan; valid CPR certification.
• Minimum 3 years of clinical experience in ambulatory care or specialty, or demonstrated competency in RN II position.

Aufgaben

• Provide triage by phone and in-office visits and educate patients.
• Lead and support clinical staff with hiring, motivation, and engagement.
• Identify at-risk patients and coordinate care to improve outcomes.
• Manage medications, test results, referrals, and prior authorizations.
• Collaborate with providers, care managers, pharmacists, and behavioral health specialists.
