Job summary

Trinity Health IHA Medical Group in Michigan is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. You will perform clinical triage, care coordination, patient education, and ongoing support to improve patient outcomes and experience.

Responsibilities include triage by phone and in-office visits, care coordination with providers and care managers, and supporting transitions of care. An active Michigan RN license and CPR certification are required.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in the State of Michigan.
• Valid CPR certification.
• 1–2 years of clinical experience in a medical or physician office preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Manage medication refills, test results, referrals, and prior authorizations.
• Support transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
• Serve as a clinical resource and leader within the care team.
• Identify preventive care needs and assist patients in navigating barriers to care.
