Job summary

Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. You will help patients navigate their healthcare journey through clinical triage, care coordination, patient education, and ongoing support that improves outcomes and enhances the patient experience.

In this role, you will assess and triage patients by phone and in-office visits, provide clinical guidance, coordinate care, manage transitions, and collaborate with providers, care

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid Michigan RN license and CPR certification.
• 1–2 years of clinical experience in a medical or physician office preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, education, and care coordination.
• Support patients through transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, and other health partners.
• Manage medication refills, test results, referrals, and prior authorizations.
