Job summary

Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team in Michigan. The RN II will perform advanced nursing tasks, conduct nurse visits, provide care coordination for complex cases, and mentor staff.

Responsibilities include triage by phone and in-office visits, patient education, and collaboration with care teams to improve outcomes and patient experience.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in the State of Michigan.
• Valid CPR certification.

Responsibilities

• Performs high-level nursing care, wellness care, specialty-specific and chronic disease education/management.
• Provides direct care with individual patients and/or populations.
• Provides care coordination for patients with complex needs.
• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support patients through transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
