Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team in Michigan. The RN II will perform at an advanced level, including nurse visits, care coordination for complex needs, and clinical leadership within the care team.

You will triage and educate patients, coordinate care, and support transitions from hospital to home while collaborating with providers and other healthcare professionals to improve outcomes and access to community

Formación

• Graduate of an accredited Registered Nurse program.
• Unrestricted RN license in Michigan and CPR certification.

Responsabilidades

• Performs high-level nursing care, wellness care, and chronic disease education/management.
• Provides direct care with individual patients and/or populations.
• Coordinates care for patients with complex needs and facilitates transitions of care.
• Assesses and triages patient needs by phone and in-office visits.
• Provides clinical guidance and patient education to support outcomes.
• Supports patients through transitions of care and post-discharge follow-up.
• Collaborates with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
