Job summary

Harbor Healthcare System in Overland Park, KS seeks a qualified RN Case Manager for an outpatient position. You will work with an interdisciplinary team to admit, assess, coordinate care, and manage discharges and pronouncements for patients and families.

Qualified candidates are a Kansas-licensed RN with 1+ year experience (hospice or home health preferred), strong communication, and reliable transportation for travel. Mon-Fri, 8:00 a.m. – 5:00 p.m., with on-call rotation.

Qualifications

• Licensed RN in Kansas required.
• 1+ years RN experience preferred; hospice/home health experience preferred.
• Strong written and verbal communication; travel with reliable transportation.

Responsibilities

• Admit and coordinate appropriate care for patients and families.
• Assess, discharge and pronouncements as part of the care team.
