Job summary

Phoenix Home Care and Hospice is seeking a Home Health Case Manager to coordinate patient care in patients' homes across Overland Park and surrounding areas. You will collaborate with physicians, LPNs, and therapy teams to develop individualized care plans aligned with orders and comprehensive initial assessments.

The role requires Kansas RN licensure, a valid driver's license, and prior home health or critical care experience.

Qualifications

• RN licensure required in Kansas or compact state.
• Experience in home health or critical care preferred.
• Strong assessment, diagnostic interpretation, and communication skills.

Responsibilities

• Manage caseload and supervise LPN visits.
• Coordinate care plans with physicians and therapy teams.
• Educate patients and families on health conditions and medications.
• Write diagnosis-driven plans of care based on orders and assessments.
