Job summary

Trinity Health is seeking an RN Care Manager to perform assessments, develop care plans, and coordinate care for High Risk members in an outpatient setting. You will collaborate with admitting, attending, and consulting physicians and other practitioners to ensure appropriate care and reduce inpatient readmissions.

The role emphasizes strong coordination across the care team, robust documentation, and adherence to NCQA standards to manage patient caseload effectively in a supportive, fast-paced

Qualifications

• Graduation from an accredited BSN Program.
• Current Michigan RN license.
• Current CPR certification.
• Valid Michigan Driver’s license.
• Three years acute care or case management experience.
• Preferred: Two years of utilization or HMO experience.

Responsibilities

• Keeps member/family members informed and requests further assistance when needed.
• Demonstrates ability to follow through with requests and communicate critical information promptly.
• Functions as liaison between administration, members, physicians and other healthcare providers.
• Interacts professionally with member/family/physicians and involves them in the care plan.
