Job summary

Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. You will perform clinical triage, care coordination, and patient education to improve outcomes and the patient experience.

Responsibilities include triage by phone and in-office visits, coordinating care, managing refills and test results, and assisting with transitions of care. You will collaborate with providers, pharmacists, care managers, and other healthcare partners to

Qualifications

• RN licensure in Michigan (unrestricted).
• CPR certification required.
• 1–2 years of clinical experience in medical/physician office preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance and care coordination.
• Support transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, and behavioral health specialists.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
