Job summary

Trinity Health IHA Medical Group in Michigan is seeking a compassionate RN to join our outpatient care team. You will assess and triage patients by phone and in-office visits, provide clinical guidance, patient education, and care coordination to improve outcomes.

You will support transitions of care, collaborate with providers, care managers, pharmacists, and other health professionals, manage refills and referrals, and help patients access community resources.

Qualifications

• RN program graduate with valid license.
• Valid, unrestricted RN license in Michigan and CPR certification.
• 1–2 years clinical experience in a medical/physician office preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support patients through transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
