Job summary

Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. You will perform clinical triage, care coordination, patient education, and follow-up, helping patients navigate their healthcare journey and improve outcomes.

Ideal candidates hold an accredited RN degree, Michigan RN license with CPR, and 1–2 years of clinical experience in a medical setting. Join a team focused on collaborative care and meaningful patient impact.

Qualifications

• Graduate of an accredited RN program.
• Valid, unrestricted RN license in Michigan with CPR certification.
• 1–2 years of clinical experience in a medical or physician office is preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support patients through transitions of care and post-discharge follow-up.
• Collaborate with providers and care team members.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
