Hospital at Home Command Center RN - Remote, 6 Day Shift

Hospital At Home is a dynamic model of care where appropriate patients transfer to the virtual unit to complete their hospitalization. This program functions like a regular inpatient unit but is set up in the patient's home, providing medical equipment, meals, and diagnostics. The Field RN provides hands-on care and collaborates with a team including Command Center RNs, MDs, and Field Therapists such as PT and OT. The role is dynamic, requiring close work with virtual colleagues to provide clinical care.

Hospital at Home manages patients meeting inpatient criteria in their homes, leading to better outcomes, higher satisfaction, fewer complications, and shorter stays. Staff report higher job satisfaction. The acuity of patients is comparable to a standard medical floor. This model suits patients who meet specific criteria and are deemed safe for home hospitalization.

Strong candidates will have excellent clinical and communication skills, be comfortable with technology, and adaptable throughout the day. Supportive colleagues await you! The role requires obtaining nursing licenses for supported states.

Required Qualifications:

• Graduation from an accredited nursing program.
• Registered Nurse License from your state of residence in our supported footprint:
• Texas, Washington, Alaska, California, Montana, Oregon, or New Mexico RN license.
• 1 year of nursing experience.
