Job summary

Trinity Health IHA Medical Group in Michigan is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. You’ll perform clinical triage, care coordination, patient education, and ongoing support that improves outcomes and enhances the patient experience.

The RN II will mentor staff and collaborate with providers and the care team. A Michigan RN license and CPR certification are required, with 2 years of ambulatory nursing experience.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in Michigan and CPR certification.
• Minimum 2 years of ambulatory nursing experience.

Responsibilities

• Performs high-level nursing care and education.
• Provides direct patient care and population health support.
• Coordinates care for patients with complex needs.
• Assesses and triages patient needs by phone and in-office.
• Collaborates with providers and care partners.
• Manages medications, test results, referrals, and prior authorizations.
