Job summary

Community Health Associates is seeking a Health Care Coordinator to support members with behavioral health services in an outpatient setting. Qualifications

• High school diploma or equivalent required.
• Minimum of six months experience with clients in a behavioral health setting.
• Valid driver's license.
• Valid CPR/First Aid certifications or ability to obtain within 30 days of hire.
• Valid Fingerprint Clearance Card or ability to obtain within 90 days of hire.
• Three-year Motor Vehicle Record that meets our auto insurance carrier's coverage requirements.

Responsibilities

• Focus on social determinants of health needs of members.
• Resource specialist to resolve social determinants of health.
• Provides pre-engagement, engagement, and re-engagement of participants via telephone, in person, telehealth and email.
• Assists in follow up contacts, outreach, and updates.
• Coordinates member care in the service area.
• Maintains confidentiality of records relating to clients' treatment.
