Job summary

Center for Human Development (CHD) in Springfield, MA is hiring a Recovery Coach for the Stimulant Treatment and Recovery Team (START). The role helps patients access medical care, connect to recovery resources, and stay engaged in care.

You will support care coordination, assist with arranging appointments, medications, and health goals, and work with inter-disciplinary staff to improve outcomes.

Qualifications

• Bilingual preferred and able to communicate with diverse patient populations.
• CPR and First Aid certifications are required or preferred.
• Experience in community health, case management, or recovery services is a plus.
• Ability to coordinate care with hospital programs and community resources.

Responsibilities

• Initiates contact with eligible patients to describe role and begin assessment.
• Connects patients to mental health and recovery resources.
• Escorts patients to programs to access care and recovery services.
• Sets and motivates patients toward health goals.
• Educates patients by phone and in person.
• Documents activities in patient records.
• Monitors progress toward goals within defined timeframes.
• Collaborates with staff on outreach, materials, and follow-up calls.
• Assists with organizing records, scheduling follow-ups, and prescriptions.
• Provides advocacy and supports access to community and hospital programs.
• Refs to care management when other issues identified.
