Job summary

H2 Health is seeking a Physical Therapist Assistant to join our Murray, Kentucky outpatient team. You will work under a licensed PT to implement treatment plans that improve mobility, strength, balance, and independence, while educating patients and caregivers on home programs.

Experience in outpatient orthopedic or post-operative rehab is a plus. You will use Scribe AI for documentation and the Raintree EMR for scheduling, with a typical 10–12 patient visits per day and a collaborative clinical

Qualifications

• Associate degree from CAPTE-accredited PTA program.
• Active licensure or eligibility to practice as PTA in the United States.
• Eligible to work in the United States.
• Experience in outpatient orthopedic, musculoskeletal, or post-operative rehab is a plus.
• Familiarity with EMR documentation and scheduling workflows is a plus.

Responsibilities

• Provide skilled physical therapy treatment under the supervision of a licensed PT.
• Implement individualized plans to improve mobility, strength, balance, and independence.
• Treat orthopedic, post-operative, musculoskeletal, and mobility-related conditions.
• Educate patients/caregivers on home programs and prevention strategies.
• Document progress and communicate updates with the clinical team.
• Leverage AI-powered documentation to optimize patient care time.
• Utilize the Raintree EMR for documentation and scheduling.
• Collaborate with therapists and staff to support patient outcomes.
• Maintain a manageable caseload with about 10–12 visits per day (full-time).
• Contribute to a positive, patient-centered clinic culture.
