H2 Health in Eddyville, KY is seeking a Physical Therapist Assistant (PTA) to join our outpatient rehabilitation team. You will deliver skilled treatments under a Physical Therapist, help patients reach functional goals, educate families, and document progress with Scribe AI and the Raintree EMR.

The caseload averages 10–12 visits per day in a collaborative, patient-first environment that supports ongoing professional growth, CEU opportunities, and competitive compensation.

Qualifications

• Associate degree from a CAPTE-accredited PTA program.
• Current Kentucky PTA license or eligibility for licensure.
• CPR certification or willingness to obtain as required.

Responsibilities

• Deliver skilled physical therapy treatments under the direction and supervision of a Physical Therapist.
• Assist patients in achieving functional goals through evidence-based therapeutic interventions.
• Provide patient and caregiver education to support continued progress outside of therapy sessions.
• Monitor patient response to treatment and communicate progress with the supervising Physical Therapist.
• Maintain accurate and timely documentation of patient care and treatment progress.
• Use Scribe AI-powered documentation to streamline documentation and maximize patient care time.
• Use the Raintree EMR platform for documentation, scheduling, and patient management.
• Manage a typical full-time outpatient caseload averaging approximately 10-12 patient visits per day.
• Collaborate with Physical Therapists and other healthcare professionals to support individualized plans of care.
• Contribute to a patient-first clinical environment focused on exceptional outcomes.
