Job summary

Endeavor Health, based in the rolling Meadows area of Illinois, seeks a Physical Therapist II for outpatient rehab services. This full‑time role offers day shifts with two evening hours and occasional Saturday coverage, focusing on one‑to‑one patient care and individualized treatment planning.

Responsibilities include evaluating patients, designing and adjusting treatment plans, documenting care, and coordinating with the care team to optimize outcomes.

Qualifications

• The completion of a degree in Physical Therapy from an A.P.T.A. accredited school, or the equivalent.
• Current Illinois licensure or eligibility.
• CPR certification through the American Heart Association.

Responsibilities

• Provide 1:1 care with no double booking.
• Assess, treat, and care for patients based on developmental stages; implement plans.
• Evaluate functional abilities using ROM, strength, balance and gait assessments.
• Establish treatment goals and modalities to regain function, increase strength, reduce pain.
• Administer treatments and instruct patients; adjust as needed for effectiveness.
• Prepare and complete patient records, billing and documentation per guidelines.
• Collaborate with physicians, nurses and staff to deliver optimal care.
• Involve patient and family in plan of care; provide education and self-directed programs.
• Supervise paraprofessionals, students and volunteers; delegate tasks as directed.
• Maintain safe environment following safety and infection control procedures.
• Contribute to in-service programs and attend mandatory trainings; pursue professional growth.
• Adhere to Northwest Community Hospital standards, policies and procedures.
