Job summary

Endeavor Health in Rolling Meadows, IL, seeks an Occupational Therapist II for Outpatient Physical Rehab Services. This per diem role offers daytime hours, with no weekends, and a focus on assessment, treatment, and patient education.

The OT will evaluate functional abilities, develop individualized plans, and collaborate with the care team to ensure quality patient outcomes in an ambulatory setting.

Qualifications

• Degree in Occupational Therapy from an AOTA-accredited program or equivalent.
• Current Illinois licensure or eligibility.
• NBCOT certification.
• CPR certification from American Heart Association.

Responsibilities

• Assess and treat patients in outpatient rehab.
• Develop and implement treatment plans to regain function and reduce pain.
• Instruct patients in performance of adjunct activities and home programs.
• Complete patient records, billing and documentation per guidelines.
• Collaborate with physicians, nursing and rehab staff for optimal care.
• Involve patient and family in the care plan, including post-discharge planning.
