Job summary

Endeavor Health seeks a Physical Therapist II for Outpatient Physical Rehab Services in Buffalo Grove, IL. This full-time role requires delivering hands-on care to patients in a one-to-one setting with no double booking and collaborating with physicians and nursing staff to optimize outcomes.

You will assess and treat patients, develop individualized plans, document care, and participate in education for patients and families while maintaining safety and quality standards.

Qualifications

• Degree in Physical Therapy from an APTA-accredited school.
• Current Illinois licensure or eligibility.
• CPR certification through the American Heart Association.

Responsibilities

• Provide 1:1 care with no double booking.
• Assess, treat, and care for patients across developmental stages.
• Evaluate functional abilities using ROM, strength, pain, balance, transfers, ambulation, and wound status.
• Establish and modify treatment plans to regain function and reduce pain.
• Prepare and complete patient records, billing, and documentation.
• Collaborate with physicians, nursing, and staff to ensure optimal patient care.
• Educate patients and families; involve them in care plans.
• Supervise paraprofessionals; delegate tasks as directed.
• Adhere to safety, infection control, and body mechanics in daily practice.
