Aurora Health Care is seeking licensed mental health professionals to provide individual, family, and group therapy across diverse client populations. The role includes crisis intervention, comprehensive assessments, and coordinated discharge planning.

You will work with the Aurora Behavioral Health Services team to ensure continuity of care and patient education. Responsibilities include documentation, treatment planning, case management, and collaboration with hospital staff.

Qualifications

• Licensed as Professional Counselor in WI or IL, or
• Licensed Marriage and Family Therapist in WI or IL, or
• Licensed Social Worker, Clinical in WI or IL.

Responsabilités

• Provide quality therapy to diverse client populations and crisis intervention.
• Develop and monitor individualized treatment plans and interventions.
• Conduct assessments and coordinate discharge planning with care teams.
• Provide case management and education to patients and families.
• Complete documentation within established timelines.
