Job summary

MEDICAL ON DEMAND LLc in Deltona, Florida seeks a Board Certified or Board Eligible Internal Medicine or Family Medicine physician for a full-time outpatient primary care role focused on adult and geriatric Medicare Advantage patients. Schedule is weekday, 8-5, with no overnight calls.

Qualifications

• Board Certified or Board Eligible — Internal Medicine or Family Medicine.
• Active Florida medical license or eligibility to obtain.
• Interest in value-based care and Medicare Advantage population medicine.

Responsibilities

• Provide comprehensive outpatient primary care to adult and geriatric Medicare Advantage patients.
• Manage chronic conditions including hypertension, diabetes, COPD, and cardiovascular disease.
• Conduct Annual Wellness Visits.
• Apply HEDIS quality measures within a value-based care model.
• Develop individualized care plans with multidisciplinary team support.
• Maintain accurate and timely clinical documentation.
