Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care to Medicare populations at our neighborhood clinics. You will work in a center supported by large care teams to focus on patient outcomes and preventive care.

Applicants should be MD/DO with board certification in Internal or Family Medicine and a current US license with DEA. Experience in geriatric populations and value-based models is a plus. 40 hours weekly, full-time.

Qualifications

• MD/DO graduate with medical license in good standing.
• Board certification in Internal or Family Medicine (or eligible).
• US work authorization and active DEA license.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medications.
• Maintain Patient EMR data via Canopy and Greenway.
• Collaborate with leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
