Job summary

Sanitas in Phoenix, AZ is seeking a Nurse Practitioner to deliver outpatient primary health care. You will report to the Medical Director, collaborate with the care team, and provide comprehensive services including diagnosis, treatment and preventive care.

Requires a Master’s Degree in Nursing, 3+ years in primary care, valid NP license in the state, DEA registration, and certification as FNP or Adult Gerontology NP; BLS and ACLS; advanced English; Spanish/Creole a plus; travel up to 5%;

Qualifications

• Master's degree from an accredited nursing program.
• 3+ years of experience in primary care.
• Valid State Nurse Practitioner license with DEA certificate.
• Certification by ANCC or AANP as a Family Nurse Practitioner or Adult Gerontology NP.
• Current BLS and ACLS certifications.

Responsibilities

• Obtain health history and perform physical examinations.
• Diagnose and direct counseling for patient treatment plans.
• Prescribe medications per state statute and guidelines.
• Maintain health records per Health Information Management policies.
• Ensure continuity of care within clinic schedules and scope of practice.
• Provide excellent patient service via in person or virtual channels.
• Refer patients to required services and review incoming reports.
