Job summary

Silver Creek Physical Therapy, a member of Therapy Partners Group, seeks an Outpatient Physical Therapist to join the team in California. You will deliver high-quality patient care, create tailored treatment plans, and guide patients toward rehabilitation goals in an outpatient setting.

Qualifications

• Doctor of Physical Therapy (DPT) degree from an accredited program.
• Current state licensure or eligibility for licensure as a Physical Therapist.
• Previous outpatient experience preferred but not required.

Responsibilities

• Conduct comprehensive assessments of patients' physical conditions, mobility, and functional abilities.
• Develop individualized treatment plans based on evaluations and patient goals, considering evidence-based practices.
• Implement various therapeutic techniques, including manual therapy, exercise prescriptions, modalities, and patient education.
• While using Raintree, maintain accurate and timely documentation of patient progress, treatment sessions, and outcomes in compliance with regulations and clinic policies.
• Educate patients and their families about rehabilitation processes, home exercise programs, and preventative care strategies.
• Regularly assess and adjust treatment plans based on patient progress and feedback.
• Stay updated on the latest research, techniques, and best practices in physical therapy through continuing education and professional growth opportunities.
