Silver Creek Physical Therapy is seeking a dedicated Outpatient Physical Therapist to join our team in the San Jose area. You will provide high-quality patient care, develop tailored treatment plans, and help patients achieve rehabilitation goals in an outpatient setting.

Qualifications

• DPT from an accredited program.
• Current state licensure or eligibility for licensure as a Physical Therapist.
• Outpatient experience preferred but not required.

Responsibilities

• Evaluate patients' physical conditions, mobility, and functional abilities.
• Develop individualized treatment plans based on evaluations and goals.
• Implement therapeutic techniques, including manual therapy and exercise prescriptions.
• Maintain documentation of progress and outcomes in compliance with policies.
• Educate patients and families about rehab processes and home programs.
• Monitor progress and adjust plans as needed.
• Engage in ongoing professional development and best practices.
