Job summary

aah in Gurnee, IL is seeking a Physical Therapist for a full-time outpatient role. This position offers a competitive hourly pay and a $5,000 sign-on bonus for eligible full-time hires. 40 hours per week.

You will interpret referrals, perform musculoskeletal and neuromuscular evaluations, develop and implement care plans, discharge planning, and ensure accurate documentation. Requires Doctorate in Physical Therapy and IL PT license; BLS preferred. No prior experience required.

• Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations using appropriate methods.
• Establishes a plan of care and implements individual treatment based on evaluation results and goals; monitors outcomes and adjusts plan as needed.
• Plans for and completes patient discharge including home programs, teaching, referrals, and transition planning; communicates with stakeholders.
• Completes documentation following department policy, including evaluations, progress notes, and discharge summaries; handles billing and scheduling as required.
• Maintains site productivity standards and adheres to billing and documentation policies.
• May supervise physical therapy assistants and rehab aides; may supervise students as applicable.
• Pursues professional growth through continuing education and department in-services.
