Job summary

Advocate Health is seeking a licensed Physical Therapist for a full-time outpatient rehabilitation position in Lake Villa, IL. The role focuses on evaluating patients, creating and adjusting individualized care plans, and promoting optimal functional outcomes within a collaborative team environment.

The position requires a Doctorate in Physical Therapy, state licensure, and BLS certification, with a 40-hour workweek, Monday–Friday, no weekends, and a $41.10–$61.65 hourly wage.

Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Licensed Physical Therapist in the state of practice.
• BLS for Healthcare Providers within 90 days (AHA).

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements individualized treatment; monitors outcomes and adjusts as needed.
• Plans for discharge including home programs, caregiver education, and community referrals.
• Documents patient care per policy, completes daily statistics, billing and scheduling; ensures compliance.
• May supervise PT assistants and rehab extenders; provides guidance and ensures patient plan of care is followed.
