BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT for a full-time outpatient home health visit opportunity in our senior living office. This office services adult and geriatric clients on a per-visit basis in Independent and Assisted Living communities throughout Delaware County.

Responsibilities:

1. Make outpatient visits to clients within designated geographic areas.
2. Perform diagnostic tests such as mobility, joint range, transfer status, ambulation patterns, strength, endurance, balance, and safety assessments.
3. Develop and implement individualized care plans, including manual therapy, gait training, and balance exercises.
4. Assess and revise care plans regularly, collaborating with nursing staff as needed.
5. Educate clients and their families on rehabilitative care and activities to promote health, safety, and independence.
6. Document observations and interventions accurately using a state-of-the-art tablet.

Qualifications:

• Current Pennsylvania Physical Therapist license.
• Graduate from an American Physical Therapy Association-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with electronic medical records preferred.
