BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT for a full-time outpatient home health visit opportunity in our senior living office. This office services adult and geriatric clients in Independent and Assisted Living communities throughout Delaware County.

As a home care Physical Therapist, you will be an integral part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities:

• Perform outpatient visits to clients within designated geographic areas.
• Conduct diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
• Develop and implement individualized care plans with manual therapeutic exercises, gait training, and other interventions.
• Assess and revise care plans regularly, collaborating with nursing staff as needed.
• Educate clients and their families on rehabilitative activities to promote health, safety, and independence.
• Document observations, interventions, and evaluations accurately using electronic medical records on a touch pad tablet.

Qualifications:

• Current Pennsylvania Physical Therapist (PT) license.
• Graduation from an APTA-approved program or accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with EMR systems preferred.
