BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time outpatient home health position. The role involves providing home visits to senior clients in Independent and Assisted Living communities throughout Delaware County.

As a home care Physical Therapist, you will be a key member of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities:

1. Make outpatient visits within designated geographic areas.
2. Perform diagnostic tests, including mobility, joint range, balance, strength, and safety assessments.
3. Develop and update individualized care plans with interventions like manual exercises, gait training, and balance activities.
4. Assess and revise care plans regularly, collaborating with nursing staff.
5. Educate clients and families on rehabilitative activities to promote health and independence.
6. Document observations and interventions accurately using electronic tablets.

Qualifications:

• Valid Pennsylvania Physical Therapist license.
• Graduation from an APTA-approved program, evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
