BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT for a full-time opportunity performing outpatient home health visits for our senior living office located in Exton, PA. This office services adult and geriatric clients on a per-visit basis in territories throughout the Coatesville area.

Responsibilities for a PT - Physical Therapist include:

1. Make outpatient visits to clients in designated geographic territories.
2. Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns, strength and endurance, balance, and safety assessments.
3. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and balance interventions.
4. Assess and revise care plans regularly, collaborating with nursing and multidisciplinary teams as appropriate.
5. Educate clients and family members on rehabilitative care and activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using a touch pad tablet.

Qualifications for a PT - Physical Therapist include:

1. Current PA Physical Therapist (PT) license.
2. At least one year of recent work experience as a PT.
3. Graduation from an American Physical Therapy Association-approved program or equivalent.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Solid computer skills; prior EMR experience preferred.
