Qualifications

• Bachelor’s Degree in Physical Therapy required.
• Licensed Physical Therapist in Illinois required.
• BLS from the American Heart Association within 90 days of hire requested.
• Masters or Doctoral Degree in Physical Therapy preferred.

Responsibilities

• Maintains primary responsibility over patient plan of care with PT/PTA collaboration.
• Treats and documents patient progress, evaluations, and discharge summaries in EMR.
• Provides patient education and materials tailored to understanding and motivation.
• Works with healthcare teams to coordinate care and ensure timely treatment.
• Participates in department initiatives to improve processes and reduce costs.
